Time: Registration 8am-9am Camp 9am -1pm
Location: Monterey Park (3rd st & Osborn)
 
*You must be a parent or legal guardian of the child in order to register.
 
Registration Instructions:
1) Please type your CHILD's first and last name
2) Type PARENT phone and email address when registering
3) Provide childs's GRADE and the POSITION that your child plays
4) Download Medical Release Form here and bring it with you on the day of the camp
5) Click SUMBIT to complete registration process
